Below you will find our new patient paperwork. This will need to be completed before we can schedule your first appointment. Please complete all paperwork in full and submit as a PDF, as we can not accept photos. Editable PDFs are below for your convience.

 

If you have more than one child, please complete both documents for each of them. Once completed, we ask that you email your paperwork to weecarerecords@hushmail.com or bring it in to our office.
